Illinois Hockey Star Justin Maciuk, 38, Leaves Behind Heartbroken Parents and Fiancée After Cancer Battle

Justin Maciuk, a former hockey star from Antioch, Illinois, tragically passed away on Wednesday, April 23, 2025, at the age of 38 after a battle with cancer.

Moreover, a fundraiser set up by his father, David ‘Dave’ Maciuk, to support the late Justin Maciuk during his battle with cancer stated that he was receiving treatment at Northwestern Hospital.

The post further revealed that the cancer had spread to many major organs in Justin Maciuk’s late body, including his brain, and that he had undergone his first chemotherapy treatment before his recent passing.

Similarly, the whole Antioch, Illinois, community mourns the untimely death of their beloved native and hockey star Justin Maciuk and extends their heartfelt condolences to his family during this difficult time.

“So incredibly sad,our friend Justin Maciuk passed away from cancer yesterday. This hits home since the same thing happened to my daughter so quickly. Justin was a friend to my family and was also one of my customers for years when i was bartending ! We would be invited graciously to his and his fiancé Ambers home! He had an infectious smile, and being around him all you can do was laugh! I remember being in a bar one night locally and we did karaoke together, and I cannot sing to save my soul! him and the whole entire crowd kept calling me Madonna all night and we just laughed and laughed ! It was one of the funniest moments with him that I will not forget! we will not forget you, Justin! May you RIP…”

The late Justin Maciuk was serving as the Vice President of Hockey Operations, co-owner and coach of JDM Sports since 2012 until his recent untimely death.

Alongside being the VP of Hockey Operations of JDM Sports, Justin Maciuk served as the director of the Chicago Blues Hockey Organization.

The Late Justin Maciuk Came From An Athletic Family

Coming from an athletic family, the late Justin Maciuk began his hockey journey from a very young age, following in his father’s footsteps.

Justin Maciuk was born on March 9, 1987, in Antioch, Illinois, to father David ‘Dave’ and mother Linda Maciuk.

His father, Dave, played minor pro hockey in Chicago with the Chicago Warriors of the USHL for years.

Dave also played for the Sault Ste. Marie Greyhounds in the NOJHL during the 1971-72 season, and played 14 games with the Soo Greyhounds in the OHA.

Moreover, Justin’s father continued his hockey journey in the Continental Hockey League, appearing for several teams, including the Chicago Cardinals and Hammond Cardinals.

He also coached hockey for the Chicago schools and currently serves as president and co-owner of JDMaciuk Consulting and Sports.

Under his father’s guidance, the late Justin Maciuk began pursuing his hockey career in high school.

He further played for several teams in USHL, including Chicago Steel and Green Bay Gamblers, and continued his hockey career while attending Concordia University and Lake Forest College, playing for the MCHA.

Moreover, the late Justin Maciuk also served as the Chicago Cougars’ assistant coach and director of scouting.

Justin Maciuk’s contribution to the sports and Illinois community and his legacy in hockey will always remain in the hearts of his family, friends, and loved ones.

In addition to his parents, David ‘Dave’ and Linda Maciuk, Justin Maciuk leaves behind his loving partner Amber Lee Knutson, siblings, and the whole family.
