Terry “Tito” Francona, a storied name in Major League Baseball, has long been admired for his leadership, insight, and resilience.

Over decades in the sport—both as a player and a manager—he’s faced the highs and lows that come with baseball’s grind.

But in recent years, his health has become a recurring subplot in his career narrative.

As Francona returns to managing with the Cincinnati Reds, questions about his medical history and physical well-being are in sharper focus than ever.

Francona Has Been Battling With Many Health Issues Throughout His Career

Francona has battled various health issues throughout his managerial career, some with serious implications.

Back in 2017, he experienced episodes of lightheadedness and underwent a catheter ablation to correct an irregular heartbeat.

He has also contended with blood clot complications and gastrointestinal issues that limited him during the 2020 season, forcing him out of action for much of the year.

In 2021, Francona underwent hip replacement surgery and had a procedure to treat a staph-infected toe, ultimately missing 63 games that season.

At the end of 2023, he planned major surgeries: replacing his right shoulder and repairing a double hernia.

When he first stepped away from Cleveland, he cited that “this lifestyle is just too difficult” and noted that each offseason had become a scramble to “get healthy for life.”

Despite stepping down, he later surfaced as the new manager of the Reds, suggesting he felt physically ready again.

However, even in 2025, he missed a game against the New York Mets due to illness, showing that health challenges continue to be a factor.

His health journey underscores the physical toll that long-term managing can inflict—and the constant balancing act between passion and bodily limits.

Terry Was Born Into An Athletic Family, Which Must Have Inspired Him For His Career Path

Terry Francona was born into a baseball family—his father, Tito Francona, was a longtime MLB player—and that legacy shaped his path.

He played parts of ten seasons in the majors (1981–1990) with teams including the Expos, Cubs, Reds, and Brewers, before transitioning to coaching and managing.

Francona’s managerial career began with the Philadelphia Phillies in 1997, followed by a prominent stint with the Boston Red Sox (2004–2011), where he broke the “Curse of the Bambino” and won World Series titles in 2004 and 2007.

He then led Cleveland’s Indians/Guardians from 2013 through 2023, becoming their all-time winningest manager and guiding them to multiple postseason appearances.

In October 2023, Francona announced he was stepping down as manager, largely referencing his declining health and the increasing difficulty of sustaining the job.

Not long after, the Cincinnati Reds brought him out of retirement to take their managerial helm, giving him a new chapter in his storied career.

Francona is widely respected not only for his tactical acumen but for his ability to manage clubhouse dynamics, player relationships, and team culture.

Off the field, his personal life has had its challenges. For instance, he and his long-time wife, Jacque, have publicly separated, and there has been media attention on the personal toll of a long baseball life.

Additional Infomation

  • He has achieved over 1,950 managerial wins, placing him among the most successful managers in MLB history.
  • During his final home game managing Cleveland, his beloved motorized scooter (which he used to ride to and from Progressive Field) was stolen and stripped just days before his farewell.
  • Francona has long been known for his ever-present wad of chewing tobacco, a personal habit that has been part of his public image.
